Job summary

Community Physicians is seeking a Patient Care Navigator to bridge patients, families, and care teams, ensuring smooth transitions across settings. The role emphasizes advocacy, EMR data entry, and coordination of post-acute care with an emphasis on geriatric patients.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited Medical Assistant program required.
• Certified Medical Assistant (CMA) preferred.
• Minimum 1–3 years of proven EMR/EHR experience and vital signs.

Responsibilities

• Collaborate with patients, families, and multidisciplinary medical professional team.
• Support transitions to next level of care, including setting up follow-up appointments.
• Perform chart creation and data entry of patient demographics, medications, and diagnoses in the EMR.
• Assist with transition clinic services, patient intake, calls, and documentation.
• Attend IDT plan of care meetings and communicate with practitioners and nurse case managers.
• Assist in collection of key quality metrics and program performance data.
