Job summary

Community Physicians is seeking a Patient Care Navigator to bridge care between medical teams, patients, and families. You will coordinate transitions, support follow-ups, and document in the EMR. This role emphasizes compassionate patient advocacy across multiple facility locations.

The position requires a CMA or CMA training, BLS certification, and 1–3 years EMR experience. Travel to various facilities is expected, with full-time hours and a comprehensive benefits package.

Qualifications

• Must have a high school diploma or equivalent.
• Graduate of an accredited Medical Assistant program is required.
• Certified Medical Assistant (CMA) is highly preferred.
• Current Basic Life Support (BLS) Certification.

Responsibilities

• Support transitions for patients and families, coordinating care across settings and setting up follow-ups.
• Perform chart creation and data entry of patient demographics, medications, and diagnoses in the EMR.
• Assist with transition clinic services, patient intake, calls, and documentation.
• Attend plan-of-care meetings and communicate with practitioners and Nurse Case Managers.
• Assist in collecting quality metrics and participate in program-improvement data.
