Job summary

Community Physicians is looking for a Patient Care Navigator to serve as a bridge between medical practitioners, staff, and patients in Wheeling, Illinois. This full-time position involves collaborating with patients and their families, managing care transitions, and performing administrative tasks like data entry within EMR systems.

You will help identify high-risk patients and ensure their smooth transitions across health settings.

Qualifications

• Must meet at least one of the following criteria: high school diploma, CMA certification, or BLS certification.
• Minimum of 1 to 3 years of experience with EMR/EHR systems.
• 5–7 years of experience in Skilled Nursing Facilities.

Responsibilities

• Collaborate with patients, families, and medical teams.
• Conduct social risk factor assessments for patients.
• Assist in collecting quality metrics and program data.
