Qualifications

• Bachelor's degree in Social Work or related field; geriatric care experience preferred.
• At least 1 year of formal (non-family setting) experience with geriatric population in community care management or related work.
• Excellent interpersonal and written/spoken communication skills; ability to work in a multicultural environment.
• Strong organizational skills and basic computer knowledge.

Responsibilities

• Perform initial and ongoing assessments with care plan development and risk reduction.
• Identify, evaluate, implement, and monitor long-term healthcare needs and manage services.
• Maintain clear, updated documentation of all contacts and services provided.
• Perform home visits and maintain a regular schedule of client contacts and visits.
• Provide access to community resources for clients and families.
• Maintain contact with family members regarding needs and services offered to a client.
• Coordinate and authorize services with service providers to meet identified needs.

Education

Bachelor's degree in Social Work, Human Services, Psychology, or related field
