Unicity Care Management in Florida seeks a Care Manager to oversee a small private-pay client caseload, coordinating services and monitoring plans of care in clients' homes across Independent Living, Assisted Living and Memory Care settings.

The role requires collaboration with referral sources, hospitals, clinicians and attorneys, providing compassionate support and timely documentation of care activities.

Qualifications

• In-depth client assessments, including medical, psychosocial, environmental, financial and family aspects.
• Perform Fall Risk Assessments, MMSE and Depression Screenings.
• Develop and coordinate care plans and services.
• Coordinate with hospitals, professionals and referral sources.

Responsibilities

• Perform in-depth client assessments across medical, psychosocial, environmental and financial domains.
• Complete Fall Risk Assessment, MMSE and Depression Screening for clients.
• Coordinate service needs and develop plans of care with clients and families.
• Conduct regular home visits to monitor status and home environment.
• Provide emotional support and counseling to clients and families.
• Coordinate with hospitals, nursing homes and other providers.
