Job summary

Qualifications

• LPN licensed in the state
• Minimum one year of geriatric nursing or rehabilitation preferred
• Knowledge of nursing theory and practice, including administration of medications
• Ability to read, write and comprehend English
• CPR certified

Responsibilities

• Holds daily shift report with nursing assistants and reviews assignments
• Makes rounds on residents at least every two hours
• Administers medications and treatments per physician orders
• Collaborates with Care Plan Coordinator to develop and update nursing care plans monthly or as conditions change
• Assists attending physician with rounds and procedures
• Transcribes, documents and implements physician orders
• Communicates with physician and family about changes in resident condition
• Evaluates nursing assistants independently for performance and records
• Documents disciplinary concerns for nursing staff and recommends actions
• Completes required routine documentation and auditing
• Participates in hiring, interviewing and orientation of nursing assistants
• Contributes to Quality Assurance and Performance Improvement (QAPI) process
