Job summary

Onyx Health Consulting seeks a licensed Practical Nurse (LPN) to oversee nursing care on an assigned unit in a Florida health facility. You will supervise nursing assistants, administer medications, and collaborate with the care team to maintain high-quality resident care.

The role requires Florida LPN licensure, CPR certification, and prefer 1 year of geriatric nursing or rehabilitation experience. The position emphasizes adherence to policies, documentation, and participation in QAPI

Qualifications

• Licensed Practical Nurse with active Florida license.
• Minimum one year of geriatric nursing or rehabilitation experience preferred.
• Knowledge of nursing theory and practice, including administration of medications.

Responsibilities

• Holds daily shift report with nursing assistants and reviews assignments.
• Makes rounds on residents at least every two hours.
• Administers medications and treatments as ordered; orders medications.
• Collaborates with Care Plan Coordinator to develop and update nursing care plans.
• Assists attending physician with rounds and procedures as needed.
• Transcribes and implements physician orders; documents changes.
• Notifies physician and family of changes in resident condition or incidents.
• Performs annual evaluations of nursing assistants and documents performance.
• Documents disciplinary concerns and recommends actions as needed.
• Completes required routine documentation and audits.
• Participates in hiring, interviewing and orientation of nursing assistants.
• Contributes to Quality Assurance and Performance Improvement (QAPI).
