Job summary

Westlake Nursing and Rehabilitation Center in Florida is seeking a compassionate Licensed Practical Nurse (LPN) to join our nursing team. The role focuses on delivering quality care for residents in a skilled nursing setting, with emphasis on collaboration and resident well-being.

Qualifications include an active Florida LPN license, graduation from an accredited PN program, CPR/BLS certification, and at least one year in long-term care. EMR experience and strong teamwork are required.

Qualifications

Active LPN license in Florida.
Graduated from an accredited Practical Nursing program.
Current CPR/BLS certification.
Minimum of one year in long-term care or skilled nursing.
Strong communication and teamwork.
Experience with EMR in a healthcare setting.

Responsibilities

Administer medications, treatments, and therapies as prescribed.
Monitor and assess residents, reporting changes to the RN and physician.
Supervise and support CNAs.
Maintain accurate and timely documentation in the EMR.
Assist with admissions, transfers, and discharges.
Collaborate with physicians, therapists, social services, and other interdisciplinary team members.
Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.
Promote resident dignity, safety, and quality of life.
Respond promptly to resident needs and emergency situations.
Participate in care planning meetings, in-service education, and quality improvement initiatives.
