Discovery Senior Living in Boynton Beach, FL, is seeking a Licensed Practical Nurse (LPN) to deliver hands-on nursing care in accordance with each resident’s care plan within a community setting.

The role emphasizes patient safety, dignity, and well-being, collaborating with an interdisciplinary team, administering medications, updating care plans, and participating in quality assurance and regulatory compliance to ensure high-quality resident care.

Qualifications

• Current Licensed Practical Nurse (LPN) license or RN license per state regulations.
• Minimum of two (2) years of licensed nurse experience.
• Experience in assisted living, long-term care, or skilled nursing preferred.

Responsibilities

• Provide nursing care to residents according to care plans, policies and procedures.
• Assess residents' physical and psychosocial status and monitor changes.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate and communicate resident care needs with physicians, families, pharmacies, and other providers.
• Support health promotion, disease prevention, and wellness initiatives in the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning and resident evaluations as required.
• Collaborate with caregivers, wellness team, and leadership to ensure high-quality care.
