Job summary

Rocky Mountain Health Care Services is seeking a licensed counselor to assess and treat participants in a multidisciplinary, person-centered setting. You will perform psychosocial and neuropsychological assessments, develop treatment plans, and participate in daily IDT meetings to coordinate care across settings.

The role requires Colorado LPC or LCSW licensure, experience with frail/elderly clients and severe mental illness, and knowledge of DSM-5/ICD-10 coding.

Qualifications

• Licensed professional counselor or licensed clinical social worker in Colorado required.
• Experience with frail/elderly populations and severe mental illness required.
• Knowledge of DSM-5/ICD-10 coding and trauma‑informed counseling preferred.

Responsibilities

• Conduct psychosocial, behavioral, and neuropsychological assessments and write professional reports.
• Develop individualized treatment plans aligned with participant goals and functioning.
• Participate in daily IDT meetings to support integrated care management.
