Deer Oaks is seeking a Licensed Professional Counselor to join our integrated clinical team in Colorado. You will provide therapy, assessments, care planning, and behavioral health support onsite at local long-term care facilities, with a Monday–Friday schedule and strong clinical and administrative support.

You will collaborate with nursing, social services, rehabilitation, and medical staff to coordinate treatment and ensure high-quality care for older adults and chronically ill residents,

Competenze

• Active, unrestricted LPC license.
• Onsite work at LTC facilities in local community.
• Strong written and verbal communication and documentation skills.
• Experience in geriatric behavioral health is a plus.

Mansioni

• Provide individual, family, and group therapy to long-term care residents.
• Perform clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with interdisciplinary teams to coordinate care and optimize resident outcomes.
• Maintain timely, compliant documentation using the designated system.
