Aufgaben

• Conduct psychosocial, behavioral, and neuropsychological assessments and write accurate reports.
• Develop treatment plans aligned with participant goals and functioning.
• Participate in daily IDT meetings and share relevant information.
• Assist care coordination between assisted living and skilled facilities.
• Collect data on health services and outcomes for research.
