Job summary

Rocky Mountain Health Care Services is seeking a licensed counselor to assess and treat participants in a multidisciplinary, person-centered setting. You will perform psychosocial and neuropsychological assessments, develop treatment plans, and participate in daily IDT meetings to coordinate care across settings.

The role requires Colorado LPC or LCSW licensure, experience with frail/elderly clients and severe mental illness, and knowledge of DSM-5/ICD-10 coding.

Qualifications

POSITION SUMMARY: Rocky Mountain Health Care Services (RMHCS) Counselors are responsible for providing assessments and individual/group counseling sessions to RMHCS participants. Counselors conduct assessments of participants mental health and cognitive functioning and develop appropriate treatment plans.
