Job summary

WelbeHealth, LLC. is seeking a Behavioral Health Specialist to join our interdisciplinary team and advocate for participants’ behavioral health needs. You will oversee a caseload, deliver therapy, and ensure proper documentation within a collaborative care setting.

You will work with Social Work, Psychiatry, and IDT partners using Collaborative Care Management to improve participant outcomes and provide training to staff. Strong licensing and extensive elder care experience are required.

Qualifications

• Master’s degree in Social Work, LMFT, or related field.
• Licensed by state board as LCSW or LMFT.
• Minimum of five (5) years of behavioral health therapy experience.
• Minimum of two (2) years with frail or elderly populations.

Responsibilities

• Conduct behavioral health assessments to determine care needs and goals.
• Participate in IDT meetings to develop discipline-specific goals and interventions.
• Deliver and document behavioral health interventions per care plans, including counseling and crisis intervention.
• Facilitate individual and group therapies (art, music therapy, psychosocial drama) to restore social roles.
• Provide training on behavioral health symptoms and interventions to WelbeHealth staff.
