Job summary

WelbeHealth is seeking a Social Worker to join a multidisciplinary team dedicated to providing comprehensive primary care and social services for frail seniors in the community. The role reports to the Social Work Manager and involves coordinating with the IDT to assess psychosocial needs, develop care plans, and facilitate transitions between care settings.

The ideal candidate has MSW training and experience with elder populations, with CPR/First Aid certification within 180 days of hire.

Qualifications

• Minimum of one year of experience working with a frail or elderly population preferred.
• Experience coordinating and facilitating care conference meetings preferred.
• CPR and First Aid certification within 180 days of hire and maintained thereafter; CPR/First Aid certification upon hire preferred.

Responsibilities

• Conduct psychosocial assessments and participate in IDT meetings to develop care plans.
• Deliver and document social work interventions, arrange resources and services, assist with care transitions, provide counseling and case management.
• Partner with the Primary Care Physician and other IDT members to guide smooth care transitions between settings.
• Initiate, coordinate, and facilitate care conference meetings to ensure high-quality care coordination.
• Provide discharge planning when participants disenroll from the program.
