Job summary

WelbeHealth, LLC. seeks an Occupational Therapist to coordinate restorative and rehabilitative OT services, working with the IDT to maximize participant independence and safety in ADLs.

The role emphasizes long-term participant relationships, collaboration with therapists and other providers, and a balanced workload with no after-hours documentation. Responsibilities include assessments, care planning, delivering interventions, and guiding therapy assistants in Functional Maintenance Programs.

Qualifications

• Bachelor’s degree in occupational therapy; Master’s preferred.
• Valid California Occupational Therapy License, or proof of license eligibility.
• Must obtain CPR and First Aid certification within 180 days of hire; maintain certification thereafter

Responsibilities

• Conduct Occupational Therapy assessments in the facility and occasionally in the home to define participant care needs, preferences, and goals
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed upon in participant care plans
• Provide active and passive exercise, muscle re-education, gait, and functional training of ADLs
• Work with Therapy Assistants, providing training and guidance to implement Functional Maintenance Programs (FMP)
