Job summary

The Behavioral Health Clinician role is with the Institute on Aging, working with On Lok PACE. You will help identify and implement behavioral health services within an IDT, ensuring integration into person-centered care plans.

You will lead assessments, treatment planning, and therapy delivery, collaborating with MSW, PCP, psychiatrists, and other disciplines to monitor outcomes and support frail elderly participants.

Qualifications

• Doctorate in Psychology (PhD) or Master’s in Social Work (MSW) required.
• California license as a Clinical Psychologist (PsyD) or LCSW must be active in the past 2 of 5 years.
• Minimum of five years of behavioral health therapy and assessment experience.
• At least one year working with the frail elderly population.

Responsibilities

• Conduct comprehensive behavioral health assessments and treatment planning.
• Deliver and document interventions including psychotherapy and crisis support.
• Facilitate individual and group therapies for participants and families.
• Collaborate with MSW, PCP and IDT to monitor interventions’ efficacy.
• Monitor responses to psychotropic medications with psychiatrists/PCPs.
• Educate and guide the IDT on trauma-informed behavioral care.
• Provide supervision to Associate Clinical Social Workers pursuing LCSW licensure.
• Document all care in the participant’s EHR per standards.
• Educate participants and support networks about available resources.
• Attend IDT and program meetings, contributing to open communication.
• All other related responsibilities as assigned.
