Job summary

On Lok seeks a licensed behavioral health clinician to evaluate and treat older adults within a multidisciplinary geriatric program. You will conduct comprehensive assessments, develop individualized care plans, and provide short-term psychotherapy while coordinating with the IDT and pharmacists to integrate care.

Responsibilities include crisis intervention, caregiver coaching, and thorough documentation in the EHR. Prior experience in long-term care or community mental health is preferred.

Qualifications

• Master's degree in Social Work, Counseling, or Psychology.
• LCSW or LMFT license/certification for California.
• Minimum 3 years counseling experience with at-risk populations or long-term care.

Responsibilities

• Perform intakes and comprehensive behavioral health assessments for older adults.
• Develop and implement individualized care plans with the IDT and participant goals.
• Provide short-term, evidence-based psychotherapy tailored to geriatric populations.
• Collaborate with IDT and pharmacists to integrate behavioral health into care plans.
• Support management of dementia-related behaviors and caregiver coaching.
• Conduct crisis assessment, intervention and safety planning.
• Maintain compliant documentation in the EHR and support quality improvement.
• Provide case management and participate in team meetings and case conferences.
