Job summary

HumanGood is seeking a Social Services Coordinator in New Jersey to organize, implement, and evaluate social services for residents, helping them live independently with the right supports. You will assess needs, coordinate services, and maintain records while ensuring HIPAA compliance and collaboration with the care team.

This full-time role offers a schedule of Monday–Friday, 9:00 AM–4:00 PM, with a pay range of $23.00–$25.00 per hour depending on experience, plus a generous benefits package.

Qualifications

• 1+ years of case management experience.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor's Degree in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred

Responsibilities

• Assess resident needs based on admission social services history, health, psychological and social needs.
• Promote a positive living experience for residents.
• Ensure confidentiality by adhering to HIPAA and all privacy standards.
• Collaborate with community team members to support residents' needs.
