Job summary

Creative Solutions in Healthcare, Inc. is seeking a full-time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will help residents improve mobility and quality of life under the supervision of a Physical Therapist.

You will administer PT treatments, monitor progress, and collaborate with an interdisciplinary team to tailor plans of care for diverse patients in a skilled nursing facility setting.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Strong communication and teamwork skills.

Responsibilities

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide skilled physical therapy services and interventions under the supervision of a Physical Therapist.
• Respond to requests for service and relay information to the Physical Therapist.
• Implement and modify patients’ plans of care under PT supervision.
• Measure and record patients’ motor function, strength, and muscle performance.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide clinical support and education to patients, families, and caregivers.
• Ensure thorough, timely documentation in line with policies and regulations.
• Attend meetings as designated by the Director of Rehab.
