Creative Solutions in Healthcare, Inc. is seeking a Full-Time Physical Therapy Assistant (PTA) to join our in-house rehab team.

The PTA will administer therapy to residents, support mobility and functional goals, and work under a Physical Therapist to implement plans of care. Ideal candidates hold a Texas PTA license, have knowledge of current PT practices, and can collaborate with the interdisciplinary team.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer medically prescribed physical therapy treatments under PT supervision.
• Provide skilled PT services and interventions per physician orders.
• Respond to service requests and relay information to the PT.
• Implement and modify plans of care under PT supervision.
• Measure and record patient motor function, strength, and performance.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Ensure timely and thorough documentation per policies and regulations.
• Attend required Rehab Director meetings.
