Job summary

Creative Solutions in Healthcare, Inc. is seeking a Full-Time Physical Therapy Assistant (PTA) to join our in-house Rehab team.

You will provide compassionate, patient-centered care to improve mobility, strength, and quality of life for residents. Responsibilities include administering prescribed treatments, working under the supervision of a Physical Therapist, documenting progress, and collaborating with the interdisciplinary team to adjust plans of care.

Qualifications

• Active and valid Texas Physical Therapy Assistant license.
• Current knowledge of physical therapy treatment practices.
• SNF or long-term care experience preferred.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Administer prescribed physical therapy treatments under PT supervision.
• Provide skilled PT services and interventions per physician orders.
• Document progress and ensure timely, compliant records.
• Collaborate with the interdisciplinary team to adjust plans of care.
• Educate patients, families, and caregivers.
