Job summary

Responsibilities

• Provides direct therapy services to patients seven days a week, including holidays.
• Evaluates patients and develops comprehensive care plans with modalities and education.
• Gathers and interprets data from medical records and family to inform care.
• Documents evaluations, tests, progress notes, discharge notes and charges.
• Communicates progress to physicians and clinical staff and coordinates treatment and discharge planning.
• Provides supervision of PT assistants and rehab staff as required.
