Job summary

Sage Creek Post-Acute seeks an experienced Staff Occupational Therapist to evaluate and treat patients, coordinate with families and physicians, and maintain comprehensive documentation.

You will train facility staff, supervise OT assistants, and participate in discharge planning while ensuring compliance with state practice acts and payer requirements. Licensure in good standing is required.

Qualifications

• Licensed Occupational Therapist in the state.
• Ability to read technical procedures and policy manuals.
• Strong communication with families, physicians and health team.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop treatment plans and obtain physician approvals.
• Treat patients per physician plan and document in medical records.
• Supervise OT Assistants and aides.
• Coordinate discharge planning and family education.
• Maintain compliance with state practice act and payer requirements.
