Sage Creek Post-Acute is seeking a Staff Physical Therapy Assistant to support the Physical Therapist in delivering patient-centered care. You will assist with treatment, document progress, and help maintain a safe, efficient therapy area.

Responsibilities include recording treatment notes, communicating patient progress with the team, and participating in in-services and discharge planning. This role emphasizes collaboration with nurses, families, and other staff to optimize outcomes.

Competenze

• Licensed as a Staff Physical Therapy Assistant in the state.
• Proficient in Casamba and Point Click Care for documentation.
• Ability to read technical procedures and policy manuals.

Mansioni

• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department and treatment area.
• Participate in Patient Care and Rehabilitation Conferences as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing with training of Restorative Aides.
• Communicate with supervisor and health team regarding patient progress.
