Hireology is seeking a dedicated Occupational Therapist (OT) to join our in-house team. You will focus on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes in a skilled nursing setting.

Responsibilities include evaluating patients, developing individualized plans, recommending adaptive equipment, and collaborating with an interdisciplinary team to ensure high-quality care.

Qualifikationen

• Active and valid Texas Occupational Therapy license.
• In good standing with all regulatory agencies and licensing boards.
• SNF or long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.

Aufgaben

• Consult with patients to evaluate their condition and impact on daily activities.
• Assess and interpret evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications as needed.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per established plan of care.
• Monitor responses and adjust interventions with the team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely per policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
