Job summary

IEP Corp in California seeks a licensed Occupational Therapist to join our skilled nursing facility team. You will evaluate and treat residents with physical, cognitive, and psychosocial impairments to help them achieve independence and improve quality of life.

Responsibilities include performing assessments, creating individualized treatment plans, providing OT interventions, documenting progress, and collaborating with physicians, nurses, and other therapists to coordinate care.

Qualifications

• Current license to practice as an Occupational Therapist in California.
• Minimum 2 years of healthcare experience, preferably in a skilled nursing facility.

Responsibilities

• Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments.
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders.
• Provide occupational therapy interventions, including therapeutic exercise, ADL training, cognitive retraining, and environmental modifications.
• Monitor and document resident progress and adjust treatment plans as necessary.
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes.
• Educate residents and family members on OT goals, plans, and home exercises.
• Maintain accurate documentation of occupational therapy services.
• Participate in team meetings and quality improvement initiatives.
• Maintain current knowledge of OT practice and regulations.
