Job summary

Hireology is seeking a dedicated Full-Time Occupational Therapist (OT) to join our in-house team. You will help residents improve independence, safety, and quality of life through meaningful daily activities and functional outcomes, following a person-centered care approach.

In this role you will evaluate patients, develop treatment plans, modify interventions with the interdisciplinary team, educate families, and ensure documentation compliance.

Qualifications

• Licensed to practice Occupational Therapy in Texas.
• SNF or long-term care experience preferred.
• Proficient in documenting with Net Health EDS.
• Strong assessment and treatment planning skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Assess and interpret evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications.
• Provide adaptive equipment training to support independence.
• Deliver treatments per the plan of care.
• Monitor patient response and adjust interventions with the rehab team.
• Educate patients, families, and caregivers.
• Collaborate with interdisciplinary team for high-quality care.
• Complete timely documentation per company policies and regulations.
• Attend required meetings with the Director of Rehab.
