Job summary

Hireology is seeking a dedicated Occupational Therapist (OT) to join our in-house team in Plano, TX. The OT will evaluate, plan, and implement person-centered care to improve residents’ independence and quality of life through meaningful daily activities and functional outcomes.

The role requires an active Texas OT license, with SNF/long-term care experience preferred and familiarity with Net Health documentation. Collaboration with an interdisciplinary team ensures high-quality patient care.

Qualifications

• Active Texas Occupational Therapy license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of current occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to evaluate their condition and its impact on daily activities.
• Assess evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per the established plan of care.
• Monitor patient progress and adjust interventions with the care team.
• Provide education and support to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely per policies and regulations.
• Attend required meetings with the Director of Rehab.
