Job summary

SanStone Health & Rehabilitation is seeking an Occupational Therapist to provide comprehensive therapy in a skilled nursing facility. You will assess, plan, organize, and implement rehabilitative programs that improve mobility and independence in ADLs for residents with physical, emotional, or developmental conditions.

The role emphasizes collaboration with therapists, nurses, social workers, and others, with rigorous documentation and adherence to regulations.

Qualifications

• Master’s or Doctoral degree in Occupational Therapy from an accredited program.
• Active North Carolina OT license.
• 1–2 years of experience in skilled nursing/long-term care preferred; new grads welcome.

Responsibilities

• Evaluate residents’ physical, cognitive, and emotional needs.
• Develop individualized treatment plans focusing on ADLs.
• Implement evidence-based interventions to promote independence.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain timely documentation of evaluations, plans, progress, and discharge summaries.
• Provide education to residents, families, and caregivers on managing conditions.
• Assist in discharge planning and home modification recommendations.
• Ensure therapy practices comply with regulations and facility policies.
