Job summary

Douglas Manor Nursing and Rehabilitation Center in Windham, CT is seeking a dedicated Licensed Practical Nurse (LPN) for full-time 3rd shift. You will deliver resident-centered care, administer medications, monitor vitals, and support activities of daily living in a collaborative team.

The ideal candidate holds a Connecticut LPN license, completed an accredited practical nursing program, maintains CPR/BLS, and has strong communication and time-management skills.

Qualifications

• Valid, unrestricted LPN license in Connecticut.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS (or equivalent) certification as required by facility policy.
• Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.

Responsibilities

• Provide direct nursing care to residents under the supervision of an RN or as directed by physician orders.
• Administer medications and treatments per orders and facility policy.
• Monitor vital signs and changes in condition.
• Assist residents with ADLs (bathing, dressing, feeding, mobility, hygiene).
• Perform basic wound care and catheter/ostomy care as allowed by policy.
• Collect specimens for lab tests as ordered.
• Document care provided and resident observations accurately in records.
• Communicate with residents, families, and the healthcare team about status and needs.
• Adhere to infection control protocols and facility standards.
