Job summary

Douglas Manor, part of Ryders Health Management, is seeking a full-time Licensed Practical Nurse (LPN) to provide compassionate, resident-centered care on the 3rd shift at our Windham, CT facility. You will administer medications, monitor vital signs, assist with ADLs, and document care in a collaborative team.

Qualifications

• Valid, unrestricted LPN license in Connecticut.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR/BLS (or equivalent) certification as required by facility policy.
• Strong interpersonal and communication skills; ability to work well with residents, families, and the care team.

Responsibilities

• Provide direct nursing care to residents under the supervision of an RN or as directed by physician orders.
• Administer medications and treatments per orders and facility policy.
• Take and record residents’ vital signs and monitor for changes in condition.
• Assist residents with activities of daily living (ADLs) as needed.
• Perform basic wound care, dressing changes, catheter or ostomy care as allowed by facility policy.
• Collect specimens for lab tests as ordered.
• Observe and document residents’ status and report changes to supervising staff.
• Maintain accurate documentation in medical records.
