WelbeHealth, LLC. is seeking Home Care Assistants to deliver hands-on personal care in participants' homes and contribute to care planning as part of the IDT. You will follow an 8-hour day with 2–5 visits, travel reimbursements, and a strong emphasis on safety and compassionate care.

Qualificações

• High school diploma or GED required.
• Current BLS/First Aid certification required.
• CNA or HHA certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start date.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsabilidades

• Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes.
• Assist participants with Instrumental ADLs, including laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes.
• Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan.
• Provide translation services as needed.
