Job summary

JWCH Institute, Inc. in Los Angeles, CA is seeking a dedicated Case Manager to support Pathway Home participants in our NENB Housing program.

This full-time role involves intake, assessment, ISP development, referrals, and ongoing coordination of health, housing, and community services to promote stability and well-being. You'll collaborate with a multidisciplinary team, advocate for participants, and document progress to meet program requirements while delivering person-centered care within

Qualifications

• Associate degree in social work, human services, public health, or related field (preferred).
• Minimum of two years of case management or related experience.
• Valid California driver's license with clean driving record.
• Experience with homelessness and EHR/data entry.

Responsibilities

• Coordinate intake, assessment, ISP development, referrals, and follow-up.
• Collaborate with healthcare providers and community partners to coordinate services.
• Advocate for participants to ensure needs and goals are addressed.
• Provide health promotion and self-management support to participants.
• Maintain documentation, reports, and monthly program data.

Education

Associate degree in social work or related field

Bachelor's degree preferred
