Job summary

CAMBA in Brooklyn, NY seeks a Case Manager to maintain client relationships, conduct intakes, and coordinate services with community resources. The role includes documenting progress, ensuring confidentiality, and supporting tenants toward self-sufficiency.

The position requires a Bachelor's degree, experience with seriously mentally ill populations, fingerprinting/CHC clearance, and flexibility to cover varying hours. Full-time with comprehensive benefits is offered.

Qualifications

• Bachelor’s degree (e.g., B.A., B.S.W.)
• Experience working with SPMI population or related experience.
• Must be fingerprinted and obtain CHC clearance.
• As a Mandated Reporter, report suspected abuse to VPCR when required.

Responsibilities

• Maintain professional relationships with clients and protect confidentiality.
• Review documentation establishing tenants' eligibility and maintain files.
• Conduct initial intake and assess tenants' needs and family needs.
• Assist tenants in attaining goals by locating community resources and making referrals.
• Document progress notes and participate in case conferences.
• Escort tenants to appointments and follow up with referrals.
