Job summary

Qualifications

Responsibilities

• Assesses grief care needs and develops plans of care.
• Provides individual and group counseling and therapeutic interventions.
• Maintains liaison with community resources and coordinates multidisciplinary care.
• Provides anticipatory grief services and team consultation as needed.
• Organizes community education, memorial services and remembrance activities.
• Maintains accurate, timely clinical documentation per guidelines.
• Participates in team meetings and process improvements.
