Job summary

Compassus is seeking a Bereavement Coordinator to oversee an organized bereavement program for patients and families during hospice care and up to a year after death. You will work within the IDT to develop care plans, assess bereavement risk, and deliver counseling and memorial services.

The role requires a strong background in grief support, excellent communication, and the ability to collaborate with nursing facilities and community resources.

Qualifications

• Bereavement experience (2+ years) preferred.
• Bachelor's degree in a relevant field preferred.
• Ability to read, write, and speak English clearly.

Responsibilities

• Participates as a member of the IDT and assists in care plan development.
• Promotes and supports bereavement risk assessment at admission and throughout care.
• Develops and implements the bereavement plan after the patient's death.
• Delivers grief support groups and memorial services as needed.
• Maintains a bereavement resources library and coordinates community resources.
