North Country Healthcare is seeking a Hospice Bereavement Coordinator & Volunteer Liaison to develop, coordinate, implement, and evaluate our bereavement and volunteer programs across the hospice services in New Hampshire. You will align with Medicare CoPs and work with the Social Worker, IDT, and agency leadership to support patients, families, and communities.

• Bachelor’s degree in Business Administration, Healthcare Management, Social Work, Psychology, Counseling, Human Services, or related field; or 8 years of relevant experience.
• CT (Thanatology) certification preferred; encouraged within 2–3 years if not held.
• GSP-C (Grief Support Practitioner) preferred.
• BLS/ACLS/PALS or Heartsaver CPR AED certification required for clinical/nonclinical staff with regular renewals.
• Valid Driver’s license and reliable transportation required.

Responsabilidades

• Coordinate hospice bereavement program in accordance with Medicare CoP.
• Provide bereavement assessments and individualized bereavement plans.
• Oversee volunteer recruitment, training, supervision, and utilization.
• Collaborate with Interdisciplinary Team (IDT) and agency leadership in care planning.
• Maintain accurate documentation in the EMR and monitor follow-up.
• Lead grief support groups and community education initiatives.
• Ensure compliance with HIPAA and regulatory requirements.
• Prepare monthly bereavement and volunteer utilization data for QAPI dashboard.
