BrightStar Care is seeking a dedicated Home Care Field Specialist to join our team in Rochester, MN. This full-time role requires traveling within a 35-mile radius to service clients in Rochester and surrounding cities, with 30 guaranteed hours per week and a 40-hour weekly availability to ensure flexible scheduling.

The position emphasizes compassionate, hands-on care, companionship, and support with activities of daily living, transportation, and care coordination with an RN Case Manager.

Qualifications

• Comfortable traveling within a 35 mile radius of Rochester
• Experience providing hands-on care (home health, assisted living, hospital setting)
• Reliable vehicle with valid driver’s license and auto insurance
• Comfortable using Hoyer lifts

Responsibilities

• Provide companionship and conversation to enhance clients’ lives
• Assist with daily activities such as meal preparation, light housekeeping, and errands
• Provide physical assistance with bathing, dressing, grooming, transferring, and ambulation
• Accompany clients to appointments or social outings and provide transportation as needed
• Monitor and report changes in health, behavior, and needs
• Coordinate with the RN Case Manager to update the plan of care

• Premium Short Shift Pay: We offer a variety of shift Lengths ranging from 1 hour to 12 hours. Our 1-hour shifts get paid at $30 per hour and hour 2-hour shifts get paid at $22.50 an hour. Any shift 3 hours or longer is paid at a base rate between $19-$22 based on experience.

Role Responsibilities:

• Act as a member of the Care coordination team to communication with the RN Case Manager to update the plan of care.

Requirements:

• A compassionate, patient, and caring nature.
• Strong interpersonal and communication skills.
• Experience providing care to clients in a hands-on environment (Home Health, Assisted Living, Group Home, Hospital Setting)
• Reliable vehicle and a valid driver’s license with Auto Insurance.
• Negative TB test or Cleared Chest-Xray.
• Availability to work 2 weekend days per month.
