Job summary

Maxim Healthcare in Phoenix is seeking a Licensed Practical Nurse (LPN) to work with an adult patient in the home who requires G-tube care; lifting is required. Shifts are Monday-Friday, 8:00am-4:00pm.

The LPN will apply the nursing process to assess, plan, and deliver skilled care under prescribed orders, monitor patient status, and coordinate with family and health care team to ensure optimal outcomes.

Qualifications

• Currently licensed as an LPN/LVN in the state where practicing.
• TB or Chest X-Ray current.
• BLS certification current.
• One year of professional nursing experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Provide skilled nursing care/services in accordance with prescribed orders on each assignment.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collect, analyze, and interpret data from healthcare members and document nursing diagnoses.

Responsibilities:
