Job summary

Community Health Resources (CHR) is seeking a licensed Therapist for the REST Center in Willimantic, CT. The role involves delivering trauma-informed, recovery-focused care to individuals experiencing mental health or substance use crises.

A Master’s degree and appropriate licensure are required. Responsibilities include assessments, treatment planning, risk evaluations, discharge planning with natural supports, and collaborative care with family and other providers.

Qualifications

• Master’s degree or higher in a related field is required.
• Licensure LMSW, LPC-A, or LMFT-A is required; other licenses may be considered.
• LCSW, LMFT, LPC, or Licensed Psychologist preferred; LADC eligibility may apply.

Responsibilities

• Provides assessment and treatment interventions consistent with the program model.
• Conducts clinical and risk assessments.
• Creates individualized, person-centered treatment plans.
• Demonstrates competency in age-related or specialty issues per population served.
• Develops discharge plans with natural supports.
• Maintains records per agency policy.
• Provides crisis assessment and intervention for clients in crisis.
• May work in office, home, residential, or community settings as needed.
• Participates in supervision, consultations, and meetings.

Education

Master’s Degree or higher in related field

LMSW, LPC-A, LMFT-A licensure required

LCSW, LMFT, LPC, Licensed Psychologist preferred
