Job summary

Community Health Resources (CHR) in Willimantic, CT is seeking a licensed therapist for the REST Center, a 24/7 crisis stabilization program. This full-time role offers a collaborative, trauma-informed environment focused on short-term assessment, stabilization, and connection to community services.

You will conduct assessments, create individualized treatment plans, and provide crisis intervention across office, home, and community-based settings, with supervision and ongoing professional

Qualifications

• Experience working with trauma, mental illness and/or substance use disorders preferred.
• Master’s Degree or higher in a related field.

Responsibilities

• Provides assessment and treatment interventions consistent with the program model.
• Conducts clinical and risk assessments.
• Collaborates with individuals to create individualized treatment plans.
• Demonstrates competency in age-related or specialty issues and developmental needs.
• Works with clients to develop appropriate discharge plans utilizing natural support.
• Maintains collaborative relationships with family members and other service providers.
• Provides crisis assessment and intervention to monitor status of clients in crisis.
• Transports clients as needed and participates in supervision and meetings.
