Job summary

Community Health Resources (CHR) in Willimantic, CT is seeking a licensed Therapist to join the REST Center team on a full-time basis. The role involves conducting assessments, developing individualized treatment plans, and delivering crisis interventions in a 24/7 community-based setting.

The successful candidate will hold a Master’s degree, LMSW/LPC-A/LMFT-A licensure, and a driver’s license. CHR offers comprehensive benefits, tuition support, and ongoing professional development opportunities.

Qualifications

• Master’s degree or higher in related field.
• Experience with trauma, mental illness and/or substance use disorders preferred.
• LMSW, LPC-A, LMFT-A required; licensing varies by program; driver’s license required.

Responsibilities

• Provides assessment and treatment interventions consistent with the program model.
• Conducts clinical and risk assessments.
• Collaborates with individuals to create individualized, person-centered treatment plans.
• Demonstrates competency in age-related or specialty issues and developmental needs for each population served.
• Works with clients to develop discharge plans utilizing natural support.
• Maintains records according to agency policy.
• Provides crisis assessment and intervention to monitor client status.
• Transports clients as required by program.
• Participates in supervision, consultations, and meetings.
