Job summary

Community Mental Health Affiliates, Inc. is seeking a full-time Case Manager for the Intensive Assertive Community Treatment Team in New Britain, CT. The role supports clients toward independent living, coordinating care across CMHA services and community resources.

Education: Bachelor's in a mental health field preferred; CT driver’s license required. Training in MANDT, First Aid, and CPR will be provided. Schedule is Monday–Friday, 8:00am–4:00pm.

Qualifications

• Bachelor's degree in a mental health field or equivalent experience.
• Minimum one year of case management experience preferred.
• Strong communication and interpersonal skills.
• Proficient in using EMR and MS Office tools.

Responsibilities

• Maintains caseload as determined by CMHA and funder.
• Provides direct care through individualized/group education and activities.
• Teaches life skills for independent living, including finances and daily living activities.
• Educates clients to use emergency/community resources for basic needs.
• Develops and reviews individualized treatment plans and documents progress.
• Ensures timely documentation per CMHA policies, including notes and plans.
• Supervises self-administration of medications when applicable.
• Collaborates with CMHA teams to coordinate services and may provide transportation.
• Performs additional duties as assigned.
