Job summary

Community Mental Health Affiliates, Inc. is hiring a full-time Case Manager for the Intensive Assertive Community Treatment Team at their New Britain, CT location. The role supports clients toward independent living, coordinates care, and ensures client safety and satisfaction.

Monday–Friday, 8:00am–4:00pm, with a starting pay of $20.00 per hour. Responsibilities include caseload management, client education, life skills development, treatment planning, documentation, and collaboration with

Qualifications

• Bachelor’s degree in a mental health-related field or equivalent experience.
• Experience in case management or related field with understanding of mental health and co-occurring conditions.
• Proficiency with Microsoft Office products (Word, Outlook, Excel, PowerPoint) and EMR systems.

Responsibilities

• Maintains caseload as determined by CMHA and funder.
• Provides direct care through individualized and group education, skill building, and activities.
• Teaches life skills to promote independent living, including finances, housing maintenance and daily living activities.
• Develops and reviews individualized treatment plans and documents progress.
• Supervises self-administration of medication when applicable and documents it.
• Collaborates with CMHA teams and external providers to ensure coordinated care; provides transportation as needed.
• Ensures confidentiality and adheres to CMHA policies and procedures.
