Qualifications

• Bachelor's degree in a mental health field preferred or equivalent experience.
• CT driver's license and reliable vehicle required.
• Willingness to obtain MANDT, First Aid, and CPR certification.
• Experience in Case Management or related field, 1+ year preferred.
• Proficient with Microsoft Office and EMR systems.

Responsibilities

• Support clients toward the goal of eventual independent living and supervise client activity and participation in the program while ensuring client safety, comfort and satisfaction.
• Coordinates care with the client throughout the CMHA system of services.
