Job summary

Community Mental Health Affiliates, Inc. (CMHA) is a private non-profit provider in New Britain, CT, inviting applications for a full-time Case Manager on the Parkview team.

The role supports clients toward independent living and supervision of client activities within our CMHA system of services. The position requires at least one year of case management experience, a Bachelor's degree in a mental health field preferred, and a Connecticut driver’s license.

Qualifications

• Bachelor’s Degree in a mental health related field preferred or equivalent experience.

Responsibilities

• Maintains appropriate caseload as determined by CMHA and funder.
• Provides direct care to clients through education, skill building, and monitoring.
• Assists clients with life skills, finances, housing maintenance, and daily living activities.
• Educates clients to access emergency and community resources.
• Develops and reviews individualized treatment plans and documents progress.
• Completes required documentation including shift reports and progress notes.
• Supervises self-administration of medication if applicable and follows policy.
