Community Mental Health Affiliates, Inc. is hiring a full-time Case Manager for IMPACT at our New Britain facility. You will support clients with mental health and co-occurring challenges, teaching recovery skills and coordinating services to promote independence.

The role requires a Bachelor's in a mental health field, a CT driver's license, and experience with EMR systems and client documentation. Monday–Friday, 8:30am–4:30pm, with competitive hourly pay.

Qualifikationen

• Bachelor’s Degree in mental health field or equivalent experience.
• Valid Connecticut driver's license and reliable vehicle.
• Experience in Case Management in mental health and co-occurring conditions.
• Experience with EMR and Microsoft Office.

Aufgaben

• Maintain an appropriate caseload as assigned.
• Provide direct care through education, skill building, and support.
• Develop and review individualized treatment plans with clients.
• Document progress notes, treatment plans, and authorizations per policy.
• Coordinate with CMHA teams and external providers; assist with transportation as needed.
