Job summary

00 RHA Health Services, LLC is hiring a Blended Case Manager for the PA Deaf Services program. The role focuses on building networks of community resources, advocating for clients, and coordinating plans to meet complex behavioral health and medical needs.

On-call crisis support is provided 24/7 as part of the team. The position requires a Bachelor’s in a human services field or an Associate degree with related credits and relevant experience, plus a valid driver’s license.

Qualifications

• Bachelor's degree in human services related field (sociology, social work, psychology, etc.).
• Associates degree with 12 related college credits and 2 years' experience in mental health/human services.

Responsibilities

• Develops service plans and treatment plans based on client needs.
• Prepares documentation including assessments, admissions, progress notes, and plans within required timeframes.
• Assists clients in gaining funding authorization for needed services and entitlements.
• Collaborates with the treatment team and community providers to coordinate care.
