Job summary

DeafHealth seeks a dedicated Case Manager to deliver hybrid case management for DeafHealth’s CareLine and related programs. You will guide clients from intake to resolution, support enrollment in Medicaid/CHIP/Marketplace, and coordinate with partners to ensure timely services.

The role requires strong ASL and English communication, ability to manage a caseload, and collaboration with staff across departments. Located in Travis County or nearby counties with up to 20% travel.

Qualifications

• Experience in case management, benefits enrollment, healthcare navigation, or related field.
• Knowledge of or willingness to learn public benefits programs (Medicaid, CHIP, Marketplace, SNAP).
• Ability to manage caseloads with escalation when needed.
• Strong relationship-building with community organizations and providers.
• Fluent ASL and written English communication.

Responsibilities

• Own each case from intake through resolution, including eligibility screening and documentation support.
• Provide in-person and virtual case management for DeafHealth clients.
• Assist with enrollment for Medicaid, CHIP, and Marketplace.
• Coordinate with partner organizations for referrals and follow-ups.
• Maintain accurate case notes and protect client confidentiality.
